Calcium tablets: carbonate, citrate and appropriate supplementation

by Natur Total Team on Aug 30, 2026
Calciumtabletten: Wann sie sinnvoll sind – sachliche Einordnung und Sicherheit

Calcium tablets should provide an appropriate supplement, not double your dietary intake on paper. Start with this question: what amount is likely missing despite suitable foods and drinks? A product with a large number may be impractical for a small gap. If calcium has been prescribed, follow the individual treatment plan.

Identify the gap before choosing the tablet

The German Nutrition Society (DGE) gives a total daily calcium reference intake of 1,000 mg for adults aged 19 and over. Start by reviewing a few ordinary days, including mineral water and fortified foods. Recurring low intake is different from one day with few dairy products. [1]

A hypothetical example: your rough assessment regularly comes to around 800 mg. This is a reason to check the estimate and possible food changes, not automatically a reason to add a 1,000 mg tablet. The accuracy of the initial figures also matters.

Calcium carbonate and calcium citrate compared

Basic differences between common forms
FormPerspectivePractical question
Calcium carbonateAbsorption depends more on stomach acid; taking it with a meal is useful.Does the intended serving fit a regular meal?
Calcium citrateAbsorption depends less on stomach acid; it can also be taken without food.Is this an advantage under your circumstances?

The best form is not identical for everyone. The declared calcium amount, tolerance and appropriate schedule matter. Read the amount of calcium itself, not only the weight of the chemical compound. [2]

What does 500 mg mean?

Germany's Federal Institute for Risk Assessment (BfR) proposes a maximum of 500 mg calcium in the recommended daily serving of a food supplement. This precautionary product assessment is neither the total intake recommendation nor a minimum amount to aim for. Clinician-guided treatment may involve a different amount. [3]

This should be distinguished from absorption of a single serving: as the calcium portion increases, the proportion absorbed tends to decrease. Smaller servings may therefore be more practical when supplementation is appropriate. This does not mean everyone needs several calcium doses daily. [2]

A fair price comparison

Hypothetical products: pack A contains 60 tablets, each providing 250 mg calcium, and costs €12. Pack B contains 60 tablets, each providing 500 mg, and costs €18. An equivalent 500 mg amount would cost 40 cents and 30 cents respectively. B is therefore cheaper per 500 mg in this calculation, but not automatically more suitable for someone needing a smaller addition.

Also record any vitamin D or other minerals included. A combination sometimes simplifies a routine but makes separate adjustment harder. Using several combination products without reviewing them does not create a clear intake plan.

Medication and tolerance

Calcium can affect the absorption of certain medicines, including levothyroxine and some antibiotics. Required intervals depend on the medicine. Have a pharmacist review the schedule with your exact products rather than applying a universal hourly rule. Constipation and bloating are possible side effects. [2]

With kidney disease, known calcium metabolism disorders or recurrent kidney stones, the decision belongs with your clinician. A symptom or laboratory value should not be treated solely with a self-selected calcium dose.

Your short shopping checklist

Before buying, write down the desired additional amount, the number of tablets you can practically manage and products already used. Then check the daily serving, form, additional ingredients and price. Keep the complete packaging so that composition and batch can be traced if questions arise. If a product is poorly tolerated, reviewing its amount and form is more useful than simply adding another supplement.

Frequently asked questions

Is citrate always better?

No. Less dependence on stomach acid can help, but it does not determine the best choice on its own.

Is the calcium compound amount the same as the calcium amount?

No. The separate nutrient declaration is what matters.

Can any calcium tablet be split?

Only according to its instructions. Not every tablet is intended to be split.

Do I always need calcium and vitamin D together?

Assess both against your intake and, where applicable, treatment plan, rather than deciding solely because a combination product exists.

Conclusion

An appropriate calcium supplement fits the additional amount needed, medication and tolerance. A clear, smaller addition can be more useful than the largest number on a pack.
